The Impact of Chronic Disease Follow-up in the Community on the Longevity of Elderly People in Chengdu, China

- Abstract
This study investigates the influence of community chronic disease follow-up on the longevity of the elderly in Chengdu, China using a case study design. With the accelerating aging population and high prevalence of chronic diseases among the elderly, which significantly impact health and life expectancy, community chronic disease follow-up emerges as a vital health management model. Taking Community A in Chengdu as an example, the study showed that the conditions of hypertensive and diabetic patients were effectively managed through community follow-up. Notably, from 2023 to 2024, there was a statistically significant increase in the number of patients adhering to regular medication for hypertension (χ2 = 10.437, P = 0.005) and diabetes (χ2 = 9.870, P = 0.007). Concurrently, a substantial decrease in complications was observed (χ2 = 5.242, P = 0.022 for hypertension and χ2 = 8.299, P = 0.040 for diabetes). These findings suggest that community chronic disease follow-up plays a positive role in improving the control levels of blood pressure and glycemic, thereby having a beneficial impact on the longevity of the elderly. The results may be applicable to other urban communities in China with similar demographic profiles.
