A Two-Stage Optimization Method for Cross-Hospital Bed Sharing Based on Multidimensional Clustering and Mixed Integer Programming

Abstract

To address the global challenge of “coexisting structural surplus and absolute shortage of medical resources”, this paper proposes a two-stage optimization framework integrating multi-dimensional clustering and mixed-integer linear programming (MILP) for hospital clustering research, aiming to optimize resource allocation through cross-hospital bed sharing. Based on data from 9,773 patients in the ICU wards of 20 Swiss hospitals, a weighted K-means clustering model is first constructed to dynamically partition hospital clusters. On this basis, a mixed-integer programming model is designed with the goal of minimizing social total costs. Empirical studies show that within a 21-day steady-state cycle, patient waiting costs are reduced by 96.2%. Sensitivity analysis reveals that a distance penalty coefficient α=2 and a complementary reward coefficient β=1,500 represent the Pareto optimal thresholds. This framework breaks through the limitations of traditional single-hospital, single-dimensional optimization and pioneers a dynamic feature fusion and multi-objective equilibrium mechanism. It provides a quantifiable tool for regional medical resource coordination and scheduling, supports governments in building hierarchical medical platforms and emergency responses to public health incidents, and its parameter adaptive mechanism can be extended to optimization scenarios of medical resources such as hospital-wide bed sharing and equipment sharing.
