Abstract

Malaysia faces rising healthcare financing challenges, particularly for chronic conditions like end-stage renal disease. Dialysis treatment costs approximately RM250 per session, with patients requiring 2-3 weekly sessions, creating significant financial burdens especially for low-income populations. While asnaf (zakat-eligible) dialysis patients receive Baitulmal assistance, increasing demand has strained these traditional funding mechanisms, creating allocation challenges for Islamic institutions. Traditional Islamic financing mechanisms, particularly waqf, present an underexplored opportunity to address healthcare financing gaps through community-based models. This study examines stakeholder perspectives on developing a waqf-based healthcare financing framework for dialysis patients in Malaysia, focusing on feasibility, acceptance, and implementation challenges. The research employed qualitative methodology through focus group discussions with purposively sampled Islamic scholars and academicians with Shariah backgrounds (n=5). Thematic analysis revealed potential for integrating three Islamic social finance instruments: zakat, waqf, and sadaqah. The proposed model utilizes zakat funds for direct treatment costs, waqf endowments for facility maintenance and infrastructure, and sadaqah contributions for operational expenses. These findings provide fundamental insights for developing sustainable healthcare financing models leveraging Islamic social finance principles to address Malaysia’s growing healthcare needs, offering a comprehensive approach to chronic disease management through community-based financing mechanisms.
