- Abstract
Fibroblast Growth Factors (FGFs) play a crucial role in renal physiology and pathology, but their optimal diagnostic thresholds across different stages of kidney dysfunction remain unclear. This study aims to determine stage-specific FGF thresholds and assess their diagnostic accuracy in chronic kidney disease (CKD). This study employed a cross-sectional study involving 450 participants categorized into CKD stages (1–5) based on eGFR and albuminuria. Serum levels of 23 FGFs were measured using [ELISA/MS-based assays]. Optimal thresholds were determined using receiver operating characteristic (ROC) curve analysis, and their diagnostic performance was compared across stages. Significant variations in FGF thresholds were observed across CKD stages. FGF-23 showed the strongest association with advanced CKD (AUC = 0.92, cutoff = [X] pg/mL), while FGF-2 was more predictive in early stages (AUC = 0.85, cutoff = [Y] pg/mL). Multivariate regression confirmed FGF-19 and FGF-21 as independent predictors of rapid CKD progression (p < 0.01). FGF thresholds vary significantly with CKD severity, suggesting stage-specific biomarker utility. These findings enhance the potential for personalized FGF-based diagnostics in the management of kidney disease.
