Abstract

Cervical cancer remains a significant public health challenge in Indonesia, marked by persistent low participation in screening programs, with an attendance rate of 10.7% against the government’s 80% target. This study investigated the association between women’s knowledge of cervical cancer risk factors and screening practices and their reasons for non-participation in screening initiatives. A correlational analytical design with a cross-sectional approach was employed. Data were collected via questionnaire from 132 participants recruited through non-probability convenience sampling and analyzed using chi-square tests. Most participants were aged 20–39 years, had a high school education, had 3–4 children, and did not use contraceptives. While knowledge levels regarding cervical cancer risk factors were generally adequate, awareness of screening protocols was moderate. Primary reasons for non-participation included low motivation, fear of screening procedures (e.g., Pap smear or IVA test), and embarrassment. Chi-square analysis revealed no significant association between women’s knowledge of cervical cancer risk factors or screening and their reasons for avoiding screening programs at the Puskesmas. The findings suggest that low screening uptake is driven not by insufficient knowledge but by psychosocial and emotional barriers, emphasizing the need for interventions addressing motivation, fear reduction, and culturally sensitive healthcare access.
