Pharmacological Approach on Efficacy and Safety Aspects of Proton Pump Inhibitors in the Elderly: A Narrative Review

- Abstract
Proton pump inhibitors (PPIs) are among the most widely utilized medications globally. Their use in elderly individuals is recommended for the management of gastrointestinal disorders, as well as for prophylactic purposes to mitigate adverse effects associated with certain medications. The prevalence of PPI use has increased significantly among elderly patients. This study aims to provide a comprehensive review of the mechanism of action, pharmacokinetics, efficacy, and safety of PPIs in the elderly population. PubMed, Google Scholar, and Cochrane databases were searched until November 2024. Original studies investigating the efficacy, safety, and pharmacological properties of PPIs were explicitly emphasized. Publications not written in English were excluded. Alterations in the pharmacokinetic characteristics of PPIs in the elderly, linked to bioavailability, plasma clearance, and reduced albumin levels, may diminish efficacy or heighten the risk of toxicity. PPIs are linked to a reduction in the incidence of first-time ischemic stroke and enhancement of nutritional status in the elderly. The safety of PPI use is linked to the risk of Clostridioides difficile infection (CDI), fractures, dementia, depression, negative inotropic effects, and increased cancer susceptibility. Clinicians must evaluate the legitimate indications prior to administering PPIs and their detrimental consequences on the health of the elderly.
