Determinants of Preventive Healthcare Utilization Among Older Adults, Evidence from Indragiri Hulu Regency, Indonesia

Abstract

The increasing elderly population and high prevalence of non-communicable diseases in Indragiri Hulu Regency have positioned preventive healthcare services as a primary priority. However, the utilization of these services among the elderly is influenced by multiple factors. This study aimed to analyze the determinant factors influencing elderly individuals’ decisions to utilize preventive healthcare services in this region. This study employed a descriptive-analytic design with a cross-sectional approach. The sample comprised 385 elderly individuals selected through purposive sampling from three primary healthcare centers (puskesmas) with the highest elderly populations. Primary data were collected using a questionnaire that measured independent variables (health insurance ownership, accessibility, family support, perception of illness, and service quality) and the dependent variable (decision to utilize services). Data were analyzed using logistic regression. The results indicated that the majority of respondents (n=385) were female (62.9%), married (67.8%), unemployed (63.1%), and possessed health insurance (93.8%). Statistical analysis revealed that accessibility ( p < 0.05 ) and family support ( p < 0.05 ) significantly influenced elderly individuals’ service utilization decisions. Elderly individuals with good accessibility were 1.292 times more likely to utilize services, while those with family support were 1.531 times more likely to do so. Health insurance ownership, perception of illness, and service quality did not demonstrate significant influence ( p > 0.05 ). The study concludes that accessibility and family support are key determinants of preventive healthcare service utilization among the elderly in Indragiri Hulu Regency. Health policies should focus on improving accessibility and strengthening family involvement. Recommended operational interventions include providing elderly-friendly transportation, implementing flexible service scheduling, and developing educational programs that engage families as health companions.
