- Abstract
Psychotic major depression (PMD) is a severe form of major depressive disorder (MDD), characterized by depressive symptoms accompanied by hallucinations and/or delusions. Schizotypal personality disorder (STPD) is a personality disorder marked by social and interpersonal deficits as well as cognitive-perceptual distortions, and it is frequently comorbid with mood disorders, thereby increasing vulnerability to depressive episodes. This case report describes a 36-year-old woman who experienced severe depressive symptoms accompanied by mood-congruent auditory and visual hallucinations, suicidal ideation, social withdrawal, and impaired daily functioning. The patient also exhibited characteristic features of STPD that had been present since adolescence, including magical thinking, social isolation, unusual perceptual experiences, and eccentric behavior. A breakup with her partner in an online romantic relationship served as the primary trigger for the depressive episode. Initial management consisted of risperidone administration and inpatient care to control psychotic symptoms and prevent suicide risk.
