Abstract

Against the backdrop of rising demands for medication management within community health systems, this study develops an intelligent pharmaceutical care model that integrates Internet of Things (IoT) architectures, artificial intelligence (AI), and cloud-based services. Drawing on theories from pharmacy informatics, smart healthcare, and community health management, the proposed framework forms a closed operational loop encompassing data acquisition, intelligent analysis, pharmacist verification, personalized intervention, and behavioral feedback. The model incorporates a multi-source data collection mechanism, an AI-driven medication-risk detection module, a knowledge-graph-enabled reasoning engine, and a digital platform that supports pharmacist decision-making, enabling real-time linkage among health monitoring signals, prescription information, and patient adherence records. Pilot implementations at two community health service centers indicate substantial performance gains over conventional service models. The intelligent system improved the identification of potential medication risks by approximately 69.2%, reduced missed-dose events by 62.7%, and lowered repetitive pharmacist workload by 22.7%. Moreover, key clinical indicators for hypertension and diabetes demonstrated favorable trends under continuous management. These findings confirm that the model strengthens medication safety and contributes to improved health outcomes for community residents. It also offers a transferable technical pathway for accelerating the digital and intelligent transformation of community-based pharmaceutical services.
