Abstract

The care economy plays a crucial role in promoting social welfare and economic sustainability, and is drawing increasing attention from economists, policymakers, and industry leaders. It encompasses both paid and unpaid caregiving activities such as childcare, elder care, and healthcare that are essential to societal well-being. As healthcare demands continue to escalate, there is a pressing need for innovative and sustainable financing models. Islamic social finance, particularly waqf (Islamic endowments), offers a promising solution by enhancing healthcare accessibility while ensuring financial resilience. This study investigates the sustainability practices of waqf-based healthcare institutions in Malaysia, with a focus on the Waqf Annur Clinic chain and the USIM Medical Specialist Clinic. Employing a qualitative research approach, in-depth interviews were conducted and analysed using NVivo software to identify key themes related to sustainability strategies. The study proposes two sustainability frameworks as viable models for waqf healthcare institutions. The first adopts a hybrid model that integrates business operations with philanthropic funding, allowing for a fee-for-service approach while ensuring reinvestment in waqf healthcare services. The second framework centres on a network of charity-based clinics, sustained through CSR partnerships and philanthropic contributions, aimed at increasing healthcare accessibility while minimising operational costs. In conclusion, this study offers an alternative, sustainable, and community-driven approach to healthcare. It highlights how the integration of the care economy and Islamic social finance can guide policymakers in developing a more inclusive and resilient healthcare system in Malaysia.
